Each patient needs permission ( Doctors Order ) to start receiving therapy with our licensed occupational therapist (OTR/L)

We need a set of forms from your doctor. This "Physician Packet" can be downloaded, printed and given to your doctor for signature.

We also need to have the patient or parent fill out the "Patient Packet" that gives us insurance information and release forms. A "Patient Bill of Rights" is included.
